CPS Athletics Student-Athlete Virtual Check-In

Even though school sports aren’t happening, you can still take care of yourself by making sure you’re exercising, eating right, staying productive and planning ahead! CPS Athletics has created a short survey to check-in to see how you’re doing every week. Answer these questions once a week and submit your survey so we can collect important information on how to help students in the long-term AND for an opportunity to be randomly selected to win cool prizes!
